The essentials

What is Mounjaro?

Mounjaro is a dual agonist weekly weight loss injection containing tirzepatide, which works through two hormone pathways - GIP and GLP-1. That dual action helps you feel fuller for longer, reduces appetite, and has delivered strong results in clinical trials.

In SURMOUNT-1, average weight loss reached 22.5% over 72 weeks maintaining a 15 mg dose.[1]

In the SURMOUNT-5 trial, tirzepatide also delivered stronger average weight loss than semaglutide (Wegovy/Ozempic).

Mounjaro (tirzepatide) comes in a pre-filled pen with four doses. Mounjaro is licensed in the UK for adults living with obesity, and for some adults who are overweight and have a related medical condition. Mounjaro works best - to lose weight - alongside healthy lifestyle changes and ongoing clinical support.

How does Mounjaro work for weight loss?

Mounjaro works through two hormone pathways - glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic peptide (GIP) - that signals to your brain that you are full. Most weight loss injections work on one pathway. Mounjaro works on both. [4]

In practical terms this means:

  • Reduced appetite: you feel less driven to eat between meals
  • Less food noise: cravings become easier to manage
  • Feeling fuller for longer: meals feel more satisfying, even smaller ones
  • Steadier blood sugar: fewer hunger spikes throughout the day

Mounjaro works best alongside healthy eating and regular physical activity, with ongoing support from your clinical team.

Is Mounjaro for weight loss or diabetes?

Initially approved for type 2 diabetes but is also effective for weight loss. It has been approved for obesity treatment in some countries, including the UK.

Abdal Alvi, Chief Clinical Officer at Simple Online Pharmacy

For many people, Mounjaro can feel genuinely life-changing. It helps reduce hunger, making weight loss feel more manageable and allowing people to regain control over eating habits and focus on long-term health, not just the number on the scales.

Clinical evidence

How much weight can you lose on Mounjaro?

Most people achieve 15-20% weight loss within the first year, with some reaching ~20-25% over 1.5-2 years on higher doses.

Long-term data shows an average maximum weight loss of around 23% over 3 years, with most people maintaining these results while staying on treatment. [3]

Clinical trial results support this. In SURMOUNT-1, average weight loss over 72 weeks reached [1]:

Real-world Mounjaro outcomes →

Body composition

Most weight lost with Mounjaro comes from fat mass rather than lean mass - roughly three quarters from fat and one quarter from lean tissue. [6] That said, it is important to actively maintain muscle through regular physical activity. This supports healthier body composition changes alongside weight loss.

Clinical data also suggests Mounjaro can support cardiovascular and metabolic health, including reduced risk of heart-failure-related outcomes in people with type 2 diabetes and established cardiovascular disease. [7]

How long does Mounjaro take to work?

Mounjaro starts working from your first dose. Most people notice a reduction in appetite and cravings within the first week. Visible changes to your weight tend to follow over the coming months, and results continue to build throughout treatment. Noticeable changes become more apparent at around month 3.

Weight loss is not always reflected immediately on the scales. How your clothes fit and how you feel are equally - if not more - useful measures of progress. Everyone responds slightly differently, and your prescriber will monitor your progress and adjust your dose when needed.

Why am I not losing weight on Mounjaro?

Weight loss outcomes depend on factors such as dose, diet, and exercise. Some people may experience slower weight loss initially before reaching a higher dose.

Can I get Mounjaro on the NHS?

Yes, Mounjaro is available on the NHS, although access is limited. It is prescribed under specialist weight management programmes for individuals with a BMI of 35+ (or 30+ with additional health conditions).

The NHS offers a 12-week weight loss plan for patients. Further information from the NHS is available for exercise and diets. As of 2025, Mounjaro is now available on the NHS, through your GP. However, the criteria are different than for private prescribing.

Is it safe to buy Mounjaro online?

You can buy Mounjaro online safely when a regulated service includes a clinical assessment, medical history review and pharmacy checks. Patients who buy Mounjaro online should look for real prescribing oversight and ongoing support.

When comparing Mounjaro UK price across providers, cost is one factor- but prescribing oversight, clinical support, and dispensing standards are equally important. A Mounjaro price comparison UK should include what is included in the service, not just the headline cost. Patients buying Mounjaro online should always ensure that a registered prescriber reviews their assessment and that a regulated pharmacy dispenses their medication. Proper clinical follow-up and ongoing support should be part of the service, not an afterthought.

How your Mounjaro is delivered and stored

Mounjaro is a cold chain medication and must be kept at controlled temperatures during transit. We ship your pen in validated temperature-controlled packaging with ice packs to maintain fridge-safe conditions for at least 48 hours.

Our temperature mapping confirms that even if the ice packs have melted on arrival, your medication has remained within the required temperature range throughout delivery.

If you are not home when your order arrives, we will attempt redelivery. If you have selected a Safeplace, your order will be left there provided the location is safe and suitable. Our packaging keeps your medication within safe temperature conditions for at least 48 hours, so a missed first delivery does not affect the quality of your medication.

Store unopened Mounjaro pens in the fridge at 2 to 8°C. Keep the pen in the original carton and do not freeze it.

Once opened, your Mounjaro pen can be stored below 30°C for up to 30 days - it does not need to go back in the fridge between uses. [4]

Weekly treatment

Mounjaro doses and titration

Mounjaro treatment starts at 2.5mg once a week, increasing gradually every 4 weeks until you reach the dose that works best for you. [4] Not everyone needs the highest dose - the goal is the lowest effective dose of Mounjaro for your individual response.

Mounjaro dosage schedule

The standard titration schedule is:

WeeksDoseNotes
Weeks 1 to 42.5mgstarting dose
Weeks 5 to 85mgstep or maintenance dose
Weeks 9 to 127.5mgstep up dose
Weeks 13 to 1610mgstep up or maintenance dose
Weeks 17 to 2012.5mgstep up dose
Week 21 onwards15mgmaximum maintenance dose

Your prescriber will increase your dose in 2.5mg steps every 4 weeks, only when needed. Each dose increase is guided by how you are responding to treatment.

The lower strengths of Mounjaro 2.5 and 5mg will not achieve the best results. While some weight loss will be observed, the primary function of these doses is to help your body adjust to the drug without experiencing serious side effects.

For a full breakdown of how dosing works and what to expect at each stage, read our Mounjaro dosage and maintenance guide.

What to do if you miss a Mounjaro dose

If you miss a dose of Mounjaro, what you do next depends on how much time has passed [4]:

  • Within 4 days of your missed dose - take it as soon as possible
  • More than 4 days have passed - skip the missed dose entirely
  • Take your next dose on your usual day
  • Never take a double dose

Mounjaro side effects

Most side effects are digestive and tend to appear early in treatment or after a dose increase. For most people they are mild to moderate and settle within the first 20 weeks.

Three-year data from SURMOUNT-1 shows no new safety signals beyond those seen at 72 weeks. [3] Side effects follow the same pattern - mostly digestive, mostly early in treatment.

Common side effects

The most common side effects of Mounjaro are [4]:

These are most noticeable during the first few weeks and after each dose increase. Starting on a low dose and increasing gradually helps reduce their impact.

Other common side effect are [4]:

  • Dizziness
  • Low blood pressure
  • Fatigue
  • Injection site reactions
  • Rash
  • Hair loss - usually temporary and more closely related to rapid weight loss than to the medication itself

Persistent nausea, vomiting, or diarrhoea can lead to dehydration. In some cases this can affect kidney function, including in people with no prior kidney problems. [4]

Stay well hydrated throughout treatment and tell your prescriber if symptoms are severe or prolonged.

The most common early side effects. For most people it improves within the first few weeks as the body adjusts.

Nausea

"Will I feel sick all the time?"

Reality: The most common early side effect, particularly when starting or increasing your dose. For most people it settles within a few days to a couple of weeks as the body adjusts.

How we help with nausea

  • Gradual dose increases to help your body adjust
  • Guidance on timing your injection to reduce impact on your day
  • Regular early check-ins to see how you're feeling
  • Dose review if symptoms persist or feel too strong

Diarrhoea & constipation

"What if my body reacts badly?"

Reality: Both are very common early on. Weight loss injections slow digestion, which can disrupt normal bowel patterns. They're usually manageable and improve as your body settles.

How we help with diarrhoea & constipation

  • Hydration and fibre guidance to keep things moving
  • Simple movement tips to support digestion
  • Clinician support if symptoms feel persistent
  • Treatment adjustment if needed

Bloating & burping

"Will I feel uncomfortable all the time?"

Reality: Bloating, gas and burping can occur as your digestive system adjusts to the medication. They're more common early in treatment and tend to ease over time.

How we help with bloating & burping

  • Guidance on foods that can reduce bloating
  • Tips on eating slowly and avoiding carbonated drinks
  • Clinician support if symptoms are affecting daily life
  • Dose review if discomfort feels too strong

Fatigue

"Why do I feel so drained?"

Reality: Fatigue is a common side effect of weight loss injections. It's not fully understood, but is likely linked to lower calorie intake, blood sugar changes, or disrupted sleep. It usually improves as your body adapts.

How we help with fatigue

  • Nutrition guidance to maintain steady energy levels
  • Hydration reminders, tiredness is often a sign of dehydration
  • Tips on sleep routine and stress management
  • Clinician review if fatigue feels significant or persistent

Serious side effects and when to get help

While the following side effects are less common they should not be ignored [4]:

Gallbladder problems - including gallstones and gallbladder inflammation

Acute pancreatitis - seek urgent help if you have severe abdominal pain with vomiting

Increased heart rate

Altered taste

Menstrual changes - some people have reported changes to their cycle or unexpected bleeding. Speak to your prescriber if this occurs

If you experience any of the following, stop taking Mounjaro and seek medical help immediately:

  • Severe stomach pain that spreads to your back
  • Persistent vomiting
  • Severe upper abdominal pain with fever
  • Signs of a serious allergic reaction

Call 999 or go to A&E if symptoms are severe, sudden, or you feel your life may be at risk.

Not sure how urgent your symptoms are?

Call 111 (or NHS 24 if you are in Scotland) - for urgent symptoms that are not life threatening. A trained adviser will assess your symptoms and direct you to the right care

Do not wait for your next scheduled appointment if you are concerned. When in doubt, call 111 first and they will advise whether you need emergency care.

Serious risks include pancreatitis, gallbladder issues, and a rare risk of thyroid tumours. Mounjaro is not recommended for individuals with a family history of medullary thyroid cancer.

Contraindications and interactions

Mounjaro is under additional safety monitoring in the UK. This means it is a newer medicine and the MHRA is collecting more data on its long-term effects. [5] It does not mean it is unsafe - it means it is being watched closely.

Do not use Mounjaro if you [4]:

  • Are allergic to tirzepatide or any ingredient in the Mounjaro pen
  • Have a personal or family history of medullary thyroid carcinoma
  • Have Multiple Endocrine Neoplasia syndrome type 2
  • Are pregnant or breastfeeding

Speak to your prescriber before starting Mounjaro if you have:

  • A history of pancreatitis
  • Severe gastrointestinal disease
  • Kidney problems
  • Diabetic retinopathy

If any of these apply to you, your prescriber will assess whether Mounjaro is appropriate or whether an alternative treatment is more suitable. If you experience a side effect, you can report it directly to the MHRA through the Yellow Card scheme. This helps build a clearer picture of how the medicine performs in real-world use.

Mounjaro can interact with some medicines and supplements. Delayed stomach emptying can affect how some medicines are absorbed, and some combinations can increase risk.

Oral contraceptive pills: Mounjaro slows stomach emptying, which can reduce how much of the pill is absorbed, making it less effective at preventing pregnancy.

Insulin, sulfonylureas and meglitinides all lower blood sugar. When combined with GLP-1 medication, they increase the risk of hypoglycaemia (blood sugar dropping too low).

Other GLP-1 medicines: Using more than one GLP-1 receptor agonist is not recommended. This can increase side effect risk without providing additional clinical benefit.

Oral HRT (hormone replacement therapy): Mounjaro may reduce absorption of the progesterone component of oral HRT. Progesterone helps protect the lining of the womb (endometrium) when used with oestrogen, so reduced absorption could lower this protection. A transdermal option (patch or gel) may be more appropriate as it bypasses the digestive system.

Supplements (ginseng, chromium): both can affect blood sugar independently, creating unpredictable combined effects.

Alcohol: can affect blood sugar and increase the risk of side effects including nausea and dizziness.

Mounjaro (tirzepatide) slows stomach emptying, which may affect how some oral medicines are absorbed. This is usually not clinically significant, but for medicines requiring stable blood levels (such as warfarin or lithium), closer monitoring may be needed when starting or adjusting treatment. If unsure of potential interactions with your current medication, speak to your doctor or one of our healthcare professionals before starting treatment.

Some medicines can interact with Mounjaro such as:

  • Anticoagulants (blood thinning medication) such as warfarin
  • Anti-diabetic medication such as sulfonylureas and insulin
  • GLP-1 receptor agonists such as Victoza

Mounjaro can be dangerous in certain conditions so it is especially important to mention if you have:

  • Heart failure
  • Aged 75 years or older
  • Severe renal (kidney) disease
  • Severe hepatic (liver) disease
  • Pancreatitis

Mounjaro does not typically cause low blood sugar unless it is taken in combination with insulin or certain other diabetes medications. If you experience symptoms like dizziness, shakiness, or weakness, seek medical advice.

For a full breakdown of side effects and how to manage them, read our Mounjaro side effects guide.

Mounjaro and contraception

If you take the oral contraceptive pill, you will need to use additional contraception when starting Mounjaro and after each dose increase.

Mounjaro slows stomach emptying, which can reduce how much of the pill is absorbed and make it less effective.

To maintain reliable contraception, use a barrier method or switch to a non-oral contraceptive for 4 weeks after starting treatment and for 4 weeks after each dose increase. [4]

Non-oral methods that are not affected in the same way include:

  • The coil
  • Implant
  • Injection
  • Patch
  • Vaginal ring

As you lose weight, fertility can increase. Reliable contraception is important throughout treatment. If the contraceptive pill is your preferred method and you do not want to change it, it is worth discussing Wegovy and the contraceptive pill with your prescriber, as it does not carry the same interaction.

Compare

Mounjaro vs Wegovy

Both Wegovy and Mounjaro are once-weekly injections prescribed for weight loss. They're effective in different ways, and the right choice depends on how your body responds, not just the headline numbers.

Mounjaro activates two hormones (GLP-1 and GIP), while Ozempic and Wegovy only activate GLP-1. Clinical trials suggest Mounjaro leads to greater weight loss, with some patients losing over 20% of their body weight.

Both results use on-treatment efficacy analyses and involved adults without type 2 diabetes alongside lifestyle support. Individual results vary. Read SURMOUNT-5.

Studies suggest that Mounjaro results in more weight loss compared to other GLP-1 medications, although individual results may vary.

Switching from Wegovy to Mounjaro

Leave one week between your last Wegovy dose and your first Mounjaro dose. Switching from 0.25 mg - 1.7 mg you must start Mounjaro at 2.5mg and retitrate gradually, however if you are switching from 2.4 mg or 7.2 mg you may be able to start on 5 mg of Mounjaro. Your prescriber will advise the best course.

During the washout period you may feel hungrier than usual - this is normal as your body adjusts.

Switching from Mounjaro to Wegovy

Leave one week between your last Mounjaro dose and your first Wegovy dose. Depending on your current Mounjaro dose, you may not need to start Wegovy at the lowest dose - your prescriber will advise the most appropriate starting point.

For full guidance on both switching routes, read our dedicated articles:

Yes, you can switch from Ozempic to Mounjaro, but you will need to restart at Mounjaro’s lowest dose (2.5 mg) to minimise side effects. Your doctor will guide you through the transition.

Mounjaro alternatives

Mounjaro is one of several prescription weight loss treatments available in the UK, and it is not always the right starting point for everyone.

Injectable treatments work by reducing appetite and slowing digestion. Mounjaro's dual GIP and GLP-1 action sets it apart from other injectable options, which is reflected in its trial data. But stronger average results do not always mean the right fit for every patient.

Wegovy - semaglutide, once weekly. Strong clinical trial results and one of the most widely used weight loss injections in the UK

Saxenda - liraglutide, once daily. A proven treatment with a longer track record, suited to patients who prefer a more gradual approach

Nevolat - liraglutide, once daily. The same active ingredient as Saxenda at a more accessible price point

For patients who prefer not to inject, weight loss pills offer an alternative route. Orlistat is currently the main licensed weight loss medication available in capsule form in the UK. It works differently to injectable treatments- rather than reducing appetite, it reduces the amount of fat your body absorbs from food.

Wegovy pill - prescription-only GLP-1 (oral semaglutide)

Foundayo - prescription-only GLP-1 (orforglipron)

Xenical - prescription strength orlistat (120mg)

Alli - lower strength orlistat (60mg), available without a prescription

Orlistat - generic prescription version

A thorough assessment is the best way to find the treatment that suits you.

Common questions

Mounjaro FAQs

Common questions about taking Mounjaro and what to expect during your treatment journey.

Can I buy Mounjaro over the counter?

No. You cannot buy Mounjaro over the counter because it's a prescription-only medicine. A prescriber needs to assess your suitability and eligibility before prescribing.

Can you buy generic Mounjaro?

No. Generic Mounjaro (tirzepatide) is not currently available. The manufacturer Eli Lilly holds the patents, and a generic can only launch once that protection expires.

Where can I buy Mounjaro in the UK?

Mounjaro is available through private clinics, online pharmacies, and select NHS weight management programmes. Always purchase from a reputable, regulated provider.

You can buy Mounjaro online through our UK registered online prescribing service. Mounjaro is the latest GLP-1 weight loss injection available in the UK, and is the most effective. It has surpassed the effectiveness of Semaglutide (Ozempic + Wegovy), with patients seeing as much as 22.5% body weight loss.

Can I drink alcohol on Mounjaro?

Moderate alcohol consumption is generally safe, although it may worsen side effects such as nausea or increase the risk of low blood sugar, particularly in diabetics.

What should I eat on Mounjaro?

While there is no strict diet, a high-protein, balanced meal plan can help minimise side effects and support weight loss. Avoid fatty, greasy, or sugary foods that may worsen nausea.

To reduce nausea, try eating smaller meals, avoid fatty foods, and keep well hydrated. If nausea is severe, consult your doctor about adjusting your dose.

What happens if I stop taking Mounjaro?

Yes, Mounjaro (tirzepatide) does not cause physical withdrawal, so it is medically possible to stop without gradually reducing your dose. [8] The medication leaves your system over a few weeks and there is no evidence that stopping suddenly causes direct harm.

As Mounjaro leaves your system, appetite and food cravings commonly return toward pre-treatment levels. For many people, some or all of the weight lost during treatment returns. [8] How you stop and consistency of healthy habits have a direct effect on the rate of weight regain.

Some of the benefits for blood pressure and cholesterol can also lessen.

Discuss timing with your prescriber: options may include stepping down to a maintenance dose first, or adjusting other medications if relevant

Increase focus: on diet and physical activity in the months after stopping

Consider: behavioural or psychological support to help manage appetite and maintain healthy habits long term

Weight regain after stopping is not a sign that treatment failed - it reflects how the medication works. For many people, ongoing treatment is the most effective way to maintain results.

If you stop taking Mounjaro, your appetite and weight may gradually return to pre-treatment levels. Many patients continue on long-term treatment to maintain their weight loss.

Can I restart Mounjaro after a break?

If you have had a break from Mounjaro, the dose you restart on depends on how long you have been off treatment.

Within 3 weeks: You can restart on the same dose as your last injection, or move to the next dose up if you tolerated your previous dose well and want to continue progressing.

After more than 3 weeks: You may need to restart at 2.5mg and re-titrate from the beginning depending on the strength last used. This gives your body time to readjust and reduces the risk of side effects. If you are unsure which dose to restart on, speak to your prescriber before placing your next order.

Can I take Mounjaro on holiday?

Yes. Keep it stored properly in an insulated travel bag with cool packs. ensure a barrier between the ice pack and the pen to avoid the ice pack freezing the medication.

Carry your medication in your hand luggage as the cargo hold can often freeze which will denature the medication.
